Why do you want to provide a foster home?



How many dogs do you currently have?



List breeds, sexes, ages and if spayed/neutered.



Do you have a kennel run or separate room (specify) that you can offer to isolate a rescue dog?



Do you understand the commitment involved, including proper feeding, medications, grooming, love and care?  (Some dogs have been neglected or abused and need extra attention.)



Will you allow prospective adopters to come visit the dog in your home (accompanied by a Pueblo Collie/Sheltie volunteer)?



Please list your veterinarian's name, location and phone number as a reference.



Are your dogs vaccinated against kennel cough?  We can provide the vaccine.



Are you able to take a special needs dog (blind, deaf, sick, behavior problems)?
